It runs pretty true to size. I ordered my normal size and it fits very well, definitely not too small.

Is it 100% cotton or a blend?

BEST ANSWER: It feels like 100% cotton The tag only said "made in india" The description said cotton weave. I really like this blouse.

If I order my normal size, will a camisole fit, or should I order up to accommodate it?

BEST ANSWER: You can order a normal size and still wear a camisole under. The camisole itself should not be oversized but form fitting. I have a white lace one that I wear with the shirt without any problem of having to size up! Enjoy....

I would not like it sheer to the point of see through. Would I need a camisole under it?

BEST ANSWER: You do need a camisole unless you don't mind if your bra shows thru. While it is not transparent, you can see even in the picture that the model has something under it. Fortunately for me, I had an ivory camisole that did not show thru as ivory, but protected my modesty! I was afraid that my ivory color cami would not work, but it is fine. ( White might be better?)

I would say no. However, I wearing a camisole makes you feel more comfortable a very light one will do the trick.
